About the Position:
 
This position will serve as the Nutrition Education Navigator for Cornell Cooperative Extension Association of Jefferson County for the 1115 Medicaid Waiver program, and as such will provide screening, navigate referrals, and implement the nutrition portion of the 1115 Medicaid waiver program for both adults and youth. This position will facilitate Medicaid 1115 waiver activities to include phone calls, home visits, virtual visits, and community educational activities. These activities will screen Medicaid members for eligibility and referral needs.  The educator will ensure participants follow through with referrals.  Finally, the position will provide direct nutrition education to eligible referrals using evidence and research-based curricula. This position will also coordinate all administrative aspects of the Medicaid 1115 Waiver program, which includes securing new partnerships to gain access to referral sources, coordinating efforts with other staff, working with Healthy Alliance, working with CCE’s Finance Department for billing and reporting aspects. This position will assist the Nutrition Program Manager with marketing efforts to promote the program.   

Required Qualifications:

• Master’s Degree.
• Registered Dietician.
• Two (2) years volunteer or work related experience, which includes training and experience to provide nutrition education.
• Experience with and understanding of rural, low-income populations.
• Valid NYS Driver’s License and the ability to meet travel requirements associated with this position.
• Ability to work flexible hours which may include evenings and/or weekends, as needed.
