We are looking for License Practical Nursing (LPN's) staff in our Jefferson County sites.   The primary purpose would be to provide health care services to persons residing in the day and residential programs, in accordance with Regulations, to act under the auspices of the Director of Nursing Services and the RN of assigned site as the primary medical advocate for the people we support; to serve as the liaison to community-based physicians and medical service providers on behalf of the residents and their Support Team and to provide for the continuity of health care through the team approach.  You would assist with the preparation of reports and attend team meetings as requested by RN. You would administer first aid and nursing instruction to the people we support as needed and provide direct support services to the people we support.

 

The position is 40 hours per week, varied and flexible to the needs of the program; which will include mornings, evenings, weekend and holidays.   There are full time and part time opportunities available.

 

You must be licensed and currently registered with the New York State Department of Education as a Licensed Practical Nurse.  Must have a valid New York State Driver’s License and an acceptable driving record.  Must be authorized to work in the United States and pass all background clearances.

 

Travel between all assigned sites is expected. Travel to conferences or trainings as assigned. 

 

This job operates in the residential or day program environment of the people we support. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines. 

 

While performing the duties of this job, the employee is regularly required to talk or hear. The employee frequently is required to stand; walk; lift; use hands to finger, handle or feel; and reach with hands and arms. Work may require lifting, pushing, or pulling in excess of 30 pounds.

Sign on bonus for Regular full and part time positions. 

Full time - $2250 paid in 4 installments.   Part time - $1125 paid in 4 installments.
