RN CASUAL CASE MANAGER
Hospice of Jefferson County is seeking an immediate casual RN Case Manager interested in making a difference for individuals with advanced, life-limiting illnesses. You will be part of a supportive interdisciplinary team focused on holistic care, comfort, symptoms management and quality of life in care settings such as: home, skilled nursing facilities and/or our 12 bed inpatient Hospice residence.
Job Duties include: The RN Case Manager will perform assessments, collaborate with members of the interdisciplinary disciplinary team and manage individualized care plans. Overall responsibilities include planning, providing and coordinating quality end of life care. Supervise and direct care provided by other nursing staff (LPN/HHA). Coverage of on-call duties as assigned.
Required Skills: Able to work independently and complete documentation accurately and in a timely fashion. Valid driver’s license and reliable transportation. Must be willing to travel throughout Jefferson County, and parts of St. Lawrence and Oswego Counties.
Job Type: Per Diem (Casual)
Pay: $32.00 - $39.00 per hour
