Sign on Bonus - $3,000 - Paid out over 2 years.

 

Wage Range- $19.23-$21.92+/Hour, Based on Education and Experience.

 

Position Type/Expected Hours Of Work – 40 hours per week, varied and flexible to the needs of the program; which may include mornings, evenings, weekends and holidays.

 

Qualifications - Must be licensed and currently registered with the New York State Department of Education as a Licensed Practical Nurse. Will accept GPN (Graduated Practical Nurse) with the expectation that the NYS Licensed Practical Nurse License is obtained in 90 days. Experience working with developmentally disabled preferred. Valid New York State Driver’s License.

 

Work Authorization/Security Clearance – Authorized to work in the United States.

 

Summary/Objective - To provide health care services to persons residing in the day and residential programs, in accordance with Regulations. To act under the auspices of a Registered Nurse employed by the Agency. To provide for the continuity of health care through the team approach.

 

Essential Functions – Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

  1. Assist the RN to maintain current health records, medication and treatment sheets for the assigned people we support.  Maintain current listings of allergies and special needs (i.e. special diets, food sensitivities, photo-sensitivity and tetanus updates).
  2. Participate in the development of medical services.
  3. Schedule medical appointments, as required by regulation and as directed by RN.  Attend appointments where significant medical issues will be addressed.
  4. Ensure that prescriptions, medications and supplies are ordered no less than weekly, maintained and documented per Agency Policies. 
  5. Monitor people we support for signs and symptoms of acute illness, communicable diseases and disease complications/exacerbation of chronic illness. Report concerns to the site RN. Complete documentation per Agency expectations.  Adhere to all Protocols/Nursing Care Plans written by the Registered Nurse.    
  6. Review T-logs for noted concerns at the beginning of each scheduled shift.  Report any concerns noted to an RN. Document status of Minor Injuries, doctor appointment, contact to Physician, the Pharmacy, Family, Nursing etc.  These notes will contain instruction to the staff as recommended/requested.
  7. Ensure physical plant and staff compliance with sanitation and infection control. 
  8. Assist with the preparation of reports and attend team meetings as requested by RN. To administer first aid and nursing instruction to the people we support and staff as needed. Provide direct support services to the people we support.
  9. Provide medical input and documentation as indicated on all reportable and serious notable occurrences/situations as it relates to allegations of abuse, injuries or mistreatments.  Report any unusual findings (i.e. bruises, marks, behaviors) to appropriate persons.
  10. Administer medications per staffing needs, at least one time weekly.
  11. In conjunction with Residence Manager and Direct Support Professionals assist with the completion of the weekly/monthly med count/order.  Will complete weekly Medication Administration Record (MAR) Review, report medication errors and document accordingly.

Supervisory Responsibility – There are no supervisory responsibilities for this position.

 

Travel - Travel between all assigned sites is expected. Travel to conferences or trainings as assigned. 

 

Work Environment - This job operates in the residential environment of the people we support. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines. 

 

Physical Demands - The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is regularly required to talk or hear. The employee frequently is required to stand; walk; lift; use hands to finger, handle or feel; and reach with hands and arms. Work may require lifting, pushing, or pulling in excess of 30 pounds.
